PLMS ATHLETIC FORMS

Each Payette Lakes Athlete must complete the required athlete forms: PLMS Athletic Policy, Insurance, and Concussion Form each school year.  Additionally, PL Athletes must have a Sports Physical on file. 

ATHLETES ARE NOT ALLOWED TO PARTICIPATE IN ANY ATHLETIC ACTIVITY UNTIL REQUIRED FORMS ARE COMPLETED AND SUBMITTED TO THE ATHLETIC DIRECTOR. 

These include:

• A Sports Physical is required for ALL athletes (Any athlete, e.g., 6th - 8th grader who has never played an extracurricular sport at PLMS).

• Returning PLMS Athletes: A physical update form is required (for returning athletes, e.g., 6th - 8th grader with a sports physical form already on file).

Please follow these steps to complete this annual requirement:

• RETURNING ATHLETES: Digitally Sign the required Athletic Forms. Returning Athletes must complete a physical update form (Returning athletes: 6th - 8th grader with a sports physical form already on file).

• NEW PLMS ATHLETES: Any athlete grade 6th - 8th who has never played an extracurricular sport at PLMS or is a new athlete to PLMS)

Sports Physical is required. Print this form and take it with you to your Sports Physical so that your physician can fill it out and sign it.

Digitally Sign the additional required Athletic Forms and return with your completed sports physical to Athletic Director.
